fix(qr-custom): ikon menu + disable sampai QRIS statis siap

- icon bi-qr-code
- menu disembunyikan bila QRIS belum aktif/payload kosong
- form & POST ditolak, tidak membuat data tanpa QR
- versi 0.6.1
